1. Required Documents:
- Application Form for Mainland Chinese Residents Entering or Exiting Taiwan (Attachment 1 – PDF)
- Attach a color passport-style photo taken within the past year (size: 4.5 cm high x 3.5 cm wide).
- Requirements: no hat, no tinted glasses, clear facial features, no obstructions, head length between 3.2 and 3.6 cm, white background, thin glossy paper.
- Photo must not be edited or composite. Applications will be rejected if the photo does not meet requirements.
- Copy of Chinese Resident ID Card:Other valid identification or relevant documents that prove identity.
- Notarized Document Proving Family Relationship:Must be authenticated and demonstrate the relationship between the applicant and accompanying relatives.
- Authorization Letter for Taiwan Immigration Agency:Must be signed and stamped by the applicant.
- Medical Declaration Form for Mainland Chinese Citizens:Must be completed in detail and signed.
- SEF (Straits Exchange Foundation) Authorization Form (Attachment 2):Must be signed and stamped by the applicant.
- Guarantee Letter (Attachment 3):Must be completed by a relative or friend in Taiwan acting as guarantor.
2. Application Procedure:
Initial Evaluation

The application begins with an initial assessment by the hospital.

If the patient is deemed suitable for treatment in Taiwan and meets the entry requirements, they may proceed with the visa process.

The patient must provide medical reports and a summary of medical records to estimate treatment costs, hospital stay, and duration of stay in Taiwan.

Family Accompaniment

Confirm accompanying family members and obtain notarized proof of relationship at a local notary office (process takes 1–2 weeks).

Currently, a spouse or up to two relatives within the third degree of kinship may accompany the patient.

If no family will accompany the patient, notarized relationship proof is not required.

Sending Notarized Documents

Request the Chinese notary office to send the notarized copy directly to SEF in Taiwan:

Send the following to your guarantor or Travel agency in Taiwan:
- Notarized family relationship certificate (original)
- Application form (one per person)
- SEF Authorization Form (signed/stamped)
- Copy of Chinese ID (one per person; for those under 16, also include household registration copy)
- Medical Declaration Form
- Immigration Agency Authorization Form (one per person)

SEF authentication fee: NT$300 per person
(For 2 people: NT$1,200 total for visa, NT$600 for SEF)

3. Guarantor Requirements in Taiwan:

Must be a relative within the second degree of kinship with household registration in Taiwan.

If none is available, a Taiwanese citizen with legal employment and household registration can act as guarantor.

Each guarantor may only support up to 3 applicants per year.
4. Next Steps for the Guarantor in Taiwan:
Authenticate the Notarized Document

Take the original to SEF for verification. Bring a copy of the Chinese resident's ID card.

After verification, notify the medical center.

Submit Documents to the Medical Center (via mail or in person):

Original notarized document (with one photocopy for the Immigration Agency)

5. Final Steps:

The hospital will notify the patient/family to transfer the estimated medical expenses to a designated account of the hospital.

Once the hospital confirms payment and all documents are ready, the agent will submit the visa application to the Immigration Agency.

申辦流程:

由醫院進行初步審核。
若病患符合來臺就醫資格及條件,方可進行後續簽證申請程序。
須提供醫療報告及病歷摘要,作為評估醫療費用、住院天數及在臺停留時間之依據。 -
親屬陪同
確認陪同來臺的親屬,至當地公證處辦理親屬關係公證(需時約1–2週)。
目前最多允許配偶或三親等以內親屬2人同行。
若無親屬陪同,則無需辦理親屬關係公證。 -

請中國公證處將公證副本直接寄送至臺灣海基會:

臺灣保證人後續作業:

持正本至海基會辦理驗證,需帶申請人之身份證影本。

最後步驟:

醫療中心通知病人或其家屬將預估之醫療費用匯入醫院指定帳戶

醫療中心確認款項及文件無誤後,由代辦人將資料送交移民署
